Photograph & Video Consent
Use this form to grant permission for photos and videos taken during the hackathon to be used on our website and social media.
How to submit a signed copy
- Download and print the PDF, sign it, then scan.
- Email the signed PDF to consent@physicshackathon.mcgill.ca.
Parental/Guardian Consent (Under 18)
This form is required for participants under 18. It covers medical authorization, photo consent, and emergency contact information.

- Email the signed PDF to consent@physicshackathon.mcgill.ca or bring it to on-site registration.
